How to apply

1. Document gathering

   Obtain confirmation letter from the accepting Slovak medical facility confirming the specific medical treatment, its duration, and costs. Obtain pre-payment receipts or proof of insurance coverage for medical costs.

2. Online application

   Fill out the Schengen visa application form specifying medical treatment as the purpose.

3. Fee payment

   Pay 90 EUR visa application fee.

4. Appointment booking

   Schedule appointment at Slovak embassy/consulate. Apply 6 months to 15 days before travel.

5. Document submission

   Submit all documents in person.

6. Biometrics

   Provide fingerprints and biometric photograph if not already in VIS.

7. Processing wait

   15 calendar days standard, up to 45 days in exceptional cases.

8. Visa collection

   Collect passport with visa sticker upon approval.
